The authors outline research strategies that may identify the possible adaptive value of a trait. But this does not solve the problem of how to decide which characteristics of living organisms require an adaptive explanation. I suggest that knowledge of the ontogenetic and phylogenetic construction of a trait facilitates the identification of features that may have been acted on by natural selection.

Background Compulsory treatments represent a legal means of imposing treatment on an individual, usually with a mental illness, who refuses therapeutic intervention and poses a risk of self-harm or harm to others. Compulsory outpatient treatment in psychiatry, also known as community treatment order, is a modality of involuntary treatment that broadens the therapeutic imposition beyond hospitalization and into the community. Despite its existence in over 75 jurisdictions worldwide, COT is currently one of the most controversial topics in psychiatry, and it (...) presents significant ethical challenges. Nonetheless, the ethical debate regarding compulsory treatment almost always stops at a preclinical level, with the different ethical positions arguing for or against its use, and there is little guidance to support for the individual clinicians to act ethically when making the decision to implement COT. Main body The current body of evidence is not clear about the efficacy of COT. Therefore, despite its application in several countries, evidence favouring the use of COT is controversial and mixed at best. In these unclear circumstances, ethical guidance becomes paramount. This paper provides an ethical analysis of use of COT, considering the principlist framework established by Ross Upshur in 2002 to justify public health interventions during the 2002–2004 severe acute respiratory syndrome outbreak. This paper thoroughly examines the pertinence of using the principles of harm, proportionality, reciprocity, and transparency when considering the initiation of COT. Conclusion Ross Upshur’s principlist model provides a useful reflection tool for justifying the application of COT. This framework may help to inform sounder ethical decisions in clinical psychiatric practice. (shrink)

An intermediate stage in Hrushovski’s construction of flat strongly minimal structures in a relational language L produces ω-stable structures of rank ω. We analyze the pregeometries given by forking on the regular type of rank ω in these structures. We show that varying L can affect the isomorphism type of the pregeometry, but not its finite subpregeometries. A sequel will compare these to the pregeometries of the strongly minimal structures.

We investigate the isomorphism types of combinatorial geometries arising from Hrushovski's flat strongly minimal structures and answer some questions from Hrushovski's original paper.

This paper develops the very basic notions of analysis in a weak second-order theory of arithmetic BTFA whose provably total functions are the polynomial time computable functions. We formalize within BTFA the real number system and the notion of a continuous real function of a real variable. The theory BTFA is able to prove the intermediate value theorem, wherefore it follows that the system of real numbers is a real closed ordered field. In the last section of the paper, we (...) show how to interpret the theory BTFA in Robinson's theory of arithmetic Q. Since Seligman's 1971 statement that the vast majority of phobias are about objects essential to the survival of a species, a multitude of laboratory studies followed, supporting the finding that humans learn to fear and detect snakes (and other animals) faster than other stimuli. Most of these studies used schematic drawings, images, or pictures of snakes, and only a small amount of fieldwork in naturalistic environments was done. We address fear preparedness theories, and automatic fast detection data from mainstream laboratory (...) data and compares it with ethobehavioural information relative to snakes, predator-prey interaction, and snakes’ defensive kinematics strikes in order to analyse their potential matching. From this analysis four main findings arose, namely that: 1) Snakebites occur when people are very close to the snake and are unaware or unable to escape the bite; 2) Human visual detection and escape response is slow compared to the speed of snake strikes; 3) In natural environments, snake experts are often unable to see snakes existing nearby; 4) animate objects in general capture more attention over other stimuli and dangerous but recent objects in evolutionary terms are also able to be detected fast. The issues mentioned above pose several challenges to evolutionary psychology-based theories expecting to find special-purpose neural modules. The older selective habituation hypothesis (Schleidt, 1961), that prey animals start with a rather general predator image from which specific harmless cues are removed by habituation might deserve reconsideration. This book examines the significantly similar, yet finally different, thinking of two nineteenth-century existentialist thinkers, Soren Kierkegaard and Friedrich Nietzsche. Its focus is on the different ways each envisioned a joyful acceptance of life - a concern they shared. Each strove to give a place to this acceptance in his picture of life, but their conceptions of it are far apart.

Levinas's ethics of other-centered service has been criticized at the theoretical level for failing to offer a conception of moral agency adequate to ground its imperative and at the practical level for encouraging self-hatred. Levinas's explicit resistance to the incorporation of the phrase "as yourself" in the Judaeo-Christian love command might seem to validate the critics' complaints. The author argues, on the contrary, that Levinas does offer a strong and compelling conception of moral agency and that his ethics, properly understood, (...) does not entail self-abnegation. Levinas's attempt to counter excessive and manipulative self-concern and self-inflation by insisting on the dependent and situational position of the self has been wrongly overinterpreted as an abandonment of the self and its just claims. The author seeks to establish a more balanced understanding by focusing attention on the "ethics of welcome," on Levinas's distinctive conception of passivity, and on the role of "the third" in all human relations. In this paper we shall introduce the variety WQS of weak-quasi-Stone algebras as a generalization of the variety QS of quasi-Stone algebras introduced in [9]. We shall apply the Priestley duality developed in [4] for the variety N of ¬-lattices to give a duality for WQS. We prove that a weak-quasi-Stone algebra is characterized by a property of the set of its regular elements, as well by mean of some principal lattice congruences. Levinas himself raises the question: "why would I feel responsible in the presence of the Face" since "we are separate ontological beings?" This questions the character of our response to the other--both in terms of agency and motivation. While the general reception of Levinas's thought has focused on his description of us as "hostage"--that is, on the moment of assignation (or assignment) by the other--I suggest that Levinas himself also, though not as directly, addresses (as he needs to) the correlative (...) moment, the moment of response. My essay reconstructs Levinas's implicit understanding of the character of our initiative in the light of his concept of Desire. I explore the different dimensions of Desire as appetite and generosity, and I argue that the "transition to moral consciousness" that Levinas wants to "justify" is one from complacent happiness to "non-complacent happiness" and can be illuminated by the notion of "passion seeking its downfall.". Apresentam-se os resultados clínicos principais de uma primeira investigação efectuada a familiares de doentes sob tratamento paliativo no domicílio da área de Lisboa, com o instrumento de medida da satisfação SERVQUAL Modificado. Dos 58 familiares/doentes que responderam ao questionário apenas uma minona estava insatisfeita (uma classe de 5 indivíduos mostra-se francamente insatisfeita), uma classe de 15 estava moderadamente satisfeita, havendo 38 individuos fortemente satisfeitos com a qualidade e prontidäo dos serviços prestados. Porém urna percentagem elevada de doentes, segundo a opinião (...) retrospectiva dos cuidadores, faleceu com sintomatologia agravada, nomeadamente dores (60%), obstipação (67%) e depressão/ansiedade (43%). Discute-se a consistência e validade do questionáno SERVQUAL Modificado, contextualizam-se os resultados no que é o standard da prática da MCP e integram-se na promoção da dignidade humana no fim da vida. We present the main clinical findings of research on palliative home care, carried out among relatives of patients in the Lisbon area. An adapted version of SERVQUAL was used. Of a total of 58 carers surveyed only a small number (5) was unsatisfied 15 were only moderately satisfied. 38 family carers were strongly satisfied with the services. Yet, a high percentage of patients, as viewed in retrospect by their relatives died with some aggravated symptoms: 60% with more pain, 67% constipation, and 43% anxiety/depression. We discuss the consistency and validity of this adapted SERVQUAL tool and the main results are contextualized beanng in mind the standard practice of palliative medicine and the pursuit of human dignity in death. Utilizam-se largamente os próprios documentos que o justificaram e os relatórios oficiais para dar uma imagem vivida, realista e técnica das dificuldades da profissionalização e reforma dos Cuidados Paliativos, mesmo quando integrada em acções de formação num serviço de um Centro de Tratamento Compreensivo do Cancro. We put forward a synopsis of the development and institutional (...) context of the Project Humanização dos Cuidados Paliativos em Contexto Domiciliário, approved and financed by the Calouste Gulbenkian Foundation. The groundwork documents are widely used as well as official reports so as to convey a lively, realistic and technical image arising from professionalizing and reforming Palliative Medicine even when it is based on in-service training programmes in an Comprehensive Cancer Center. The impact of COVID-19 on chronic pain in non-infected vulnerable South American subjects is unknown. Healthcare workers are at increased risk for CP. During the pandemic, many HCWs with CP kept working. Knowing how cognition is affected by CP in these subjects is an important subject for work safety. The attention domain has a pivotal role in cognition. Previously, the Continuous Visual Attention Test was applied to detect specific attention deficits in fibromyalgia patients. The present investigation described CP prevalence in (...) non-infected Brazilian HCWs during the pandemic and assessed HCWs’ attentional performance with the aid of the CVAT. This study was carried out at a reference University Hospital in Rio de Janeiro, Brazil. HCWs of both sexes, aged 20 or older, were interviewed from August to December 2020. A 90-second version of the CVAT was performed. The average reaction time to correct responses and the respective intraindividual reaction time variability for correct responses to target was determined. Omission and commission errors were also calculated. Then, for each participant we calculated the Z-scores of the CVAT variables based on the distribution of CVAT performance of 211 healthy subjects. HCWs with Z-scores > 1.64 were classified as significantly impaired. From the 154 selected HCWs, 72 reported CP during the pandemic. Post hoc ANCOVAs showed that the average correct VRT was significantly higher in the CP group than in the non-CP group. The percentage of participants with impaired VRT performance was 30% in the CP group and 16% in the non-CP group. The difference between these two propositions reached significance. As VRT is associated with the sustained-attention subdomain, our data suggest that this subdomain is disrupted in the CP group. (shrink)

Most studies investigating the relationship between cultural constructs and ethical perception have focused on individual- and societal-level values without much attention to other type of cultural constructs such as social beliefs. In addition, we need to better understand how social beliefs are linked to ethical perception and the level of analysis at which social beliefs may best predict ethical perceptions. This research contributes to the cross-cultural ethical perception literature by examining the relationship of individual-level social cynicism belief, one of five (...) universally endorsed social beliefs, together with individual social dominance orientation and the perception of unethical behavior. By means of two studies, we examine these relationships across societies that significantly differ on societal-level social cynicism belief. Using 371 business students from Russia and the U.S. in Study 1 and 268 professionals from Portugal and the U.S. in Study 2, we found that individual-level social cynicism belief was positively associated with social dominance orientation. Social dominance orientation, in turn, mediated the relationship between individual social cynicism belief and the perception of unethical behavior. Although we found significant societal-level differences in social cynicism belief in both studies, the relationships between individual-level social cynicism belief, social dominance orientation, and the perception of unethical behavior were structurally equivalent across societies in both studies, suggesting that societal-level differences did not significantly affect these relationships. Implications for cross-cultural business ethics research and practice are discussed. (shrink)

IntroductionCardiology is characterized by its state-of-the-art biomedical technology and the predominance of Evidence-Based Medicine. This predominance makes it difficult for healthcare professionals to deal with the ethical dilemmas that emerge in this subspecialty. This paper is a first endeavor to empirically investigate the axiological foundations of the healthcare professionals in a cardiology hospital. Our pilot study selected, as the target population, cardiology personnel not only because of their difficult ethical deliberations but also because of the stringent conditions in which they (...) have to make them. Therefore, there is an urgent need to reconsider clinical ethics and Value-Based Medicine. This study proposes a qualitative analysis of the values and the virtues of healthcare professionals in a cardiology hospital in order to establish how the former impact upon the medical and ethical decisions made by the latter.ResultsWe point out the need for strengthening the roles of healthcare personnel as educators and guidance counselors in order to meet the ends of medicine, as well as the need for an ethical discernment that is compatible with our results, namely, that the ethical values developed by healthcare professionals stem from their life history as well as their professional education.ConclusionWe establish the kind of actions, communication skills and empathy that are required to build a stronger patient-healthcare professional relationship, which at the same time improves prognosis, treatment efficiency and therapeutic adhesion. Much has been made of the Kierkegaardian flavour of Wittgenstein's thought on religion, both with respect to its explicit allusions to Kierkegaard and its implicit appeals. Even when significant disparities between the two are noted, there remains an important core of de facto methodological agreement between them, addressing the limits of theory and the dispelling of illusion. The categories of ‘nonsense’ and ‘paradox’ are central to Wittgenstein's therapeutic enterprise, while the categories of ‘paradox’ and the ‘absurd’ are central to much (...) of Kierkegaard's attempt to dispel religious illusion. Writing of how the ‘urge to thrust against the limits of language’ yields ‘nonsense’, Wittgenstein explicitly appealed to Kierkegaard: ‘Kierkegaard, too, recognized this thrust and even described it in much the same way ’. 1 I want to consider whether Kierkegaard's category of paradox of the absurd is assimilable to Wittgenstein's view of nonsense and paradox. I shall argue that a consideration of Wittgenstein's view of paradox can highlight contrasting strands in Kierkegaard's writings on religious faith, strands which take paradox more or less strictly – in particular, it can clarify several different opinions concerning the status of religious claims. My exploration will bring to the fore some implications of the attempt to make room, in the religious employment of language, for a ‘higher understanding’ of truths which we are said to be able to grasp but cannot express. Søren Kierkegaard and John Henry Newman have starkly opposed formulations of the relation between faith and reason. In this essay I focus on a possible convergence in their respective understandings of the transition to religious belief or faith, as embodied in metaphors they use for a qualitative transition. I explore the ways in which attention to the legitimate dimension of discontinuity highlighted by the Climacan metaphor of the ‘leap’ can illuminate Newman's use of the metaphor of a ‘polygon inscribed in (...) a circle’, as well as the ways in which Newman's metaphor can illuminate the dimension of continuity operative in the Climacan appreciation of qualitative transition.
